National patient safety alert – harm from incorrect recording of penicillin allergy as penicillamine allergy

By Eric November 28, 2025

In a crucial move to enhance patient safety, NHS England’s National Patient Safety team, in collaboration with esteemed organizations such as the Royal Pharmaceutical Society, the Royal College of Physicians, and the Royal College of General Practitioners, has issued a joint National Patient Safety Alert. This alert highlights a significant risk associated with the inadvertent misrecording of patients’ penicillin allergies as penicillamine allergies within electronic prescribing and medicines administration (EPMA) systems. The potential consequences of this mix-up could lead to inappropriate prescribing practices and, ultimately, harm to patients who may be allergic to penicillin but not to penicillamine, a medication used primarily for conditions like rheumatoid arthritis and Wilson’s disease.

The alert serves as a critical reminder for healthcare professionals to exercise heightened vigilance when documenting allergy information in electronic health records. Mislabeling allergies can result in serious adverse reactions, as patients who are allergic to penicillin may be mistakenly prescribed penicillamine or other medications that could trigger an allergic response. The NHS emphasizes the importance of clear communication and verification of allergy histories to prevent such errors. Healthcare providers are urged to review their EPMA systems, ensure accurate documentation, and educate staff on the distinctions between various drug allergies. This proactive approach is essential not only for safeguarding patient health but also for maintaining the integrity of clinical decision-making processes.

As part of the alert, the NHS has provided guidance on best practices for documenting allergies, including the necessity of using standardized terminology and ensuring that allergy records are easily accessible and comprehensible. This initiative underscores the ongoing commitment of the NHS and its partners to prioritize patient safety and improve the quality of care delivered to individuals across the healthcare system. By addressing this specific risk, the alert aims to foster a culture of safety and diligence among healthcare professionals, ultimately leading to better health outcomes and increased trust in the healthcare system.
